Bound application forms (both those accepted and those rejected) containing the following information: name; place and date of birth; names and nationalities of father and mother; occupation or profession and pecuniary condition of father; post office address; nearest railroad station; names of brothers and sisters; any deaf and dumb, blind, or "idiotic" brothers and sisters or other relatives; blood relation between parents; cause of deafness (and examining doctor's note); had applicant been to school; what sounds can applicant hear; physical, mental, and moral condition; vaccination; what diseases applicant has had; church preference; course of study desired; trade preferred; and dates of application and admission.
